What sarcopenia is
Muscle mass peaks at 30 and falls afterwards by about one percent a year, from 60 by two to three. Strength vanishes faster than mass because the fast muscle fibers go first. Sarcopenia is the point at which mass and strength are so low that daily life and safety suffer: stairs, getting up, carrying groceries, keeping balance. It affects one in ten over 60 and one in three over 80 and is the strongest predictor of needing care. It is not a disease of age but of disuse, and therefore reversible.
Three tests that show it early
Grip strength with a dynamometer that family doctors and physiotherapists have: under 27 kilos for men and 16 for women is too little. Rising from a chair five times without hands: more than 15 seconds is a warning sign. Walking speed: under 0.8 meters per second, so more than five seconds for four meters, means limited. Muscles grow at any age
The classic study by Maria Fiatarone with nursing home residents between 86 and 96 showed strength gains of 174 percent and faster walking speed after eight weeks of strength training. Since then dozens of studies have confirmed: the old muscle responds to resistance just like the young one, only a little more slowly and with more protein need. The anabolic resistance of age, meaning the weaker response to protein, can be overcome with a higher dose per meal: 35 to 40 grams instead of 25.
The program from 60
Two strength sessions a week, each with six exercises: squat or rising from a chair with weight, hip raise, rows, press, lunges, carry. Two to three sets of eight to twelve reps, the last two hard. Machines in the gym are safe for beginners, dumbbells and bands at home suffice. Plus balance: one-leg stand while brushing teeth, tandem walk. And walking, briskly, 30 minutes, for the endurance muscle needs.
Protein, creatine, vitamin D, sleep
1.2 to 1.6 grams of protein per kilo, spread over three meals of 30 to 40 grams each, breakfast included. Creatine monohydrate, three to five grams daily, demonstrably amplifies the training effect in older people. Vitamin D in the target range, because deficiency weakens muscle strength and balance. Is strength training at 70 dangerous?
No, the weakness is dangerous. With medical clearance for heart problems, clean technique and slow progression, strength training in old age is the safest and most effective measure against falls and care dependence.
Is walking enough?
For heart and metabolism yes, for muscle no. Walking preserves leg endurance but no strength and no muscle mass in the upper body. Resistance is irreplaceable.
How much protein at 70?
Rather 1.5 grams per kilo than 1.2, because the old muscle responds more weakly to protein. With kidney weakness the doctor’s limit applies.
