What VO2 max is
Maximum oxygen uptake, in milliliters per kilo of body weight per minute, describes how much oxygen lungs, heart, blood and muscles can process together at maximum effort. It is the measure of the performance of the whole cardiovascular system. From 30 it falls by about 10 percent per decade if nobody trains, and in old age it decides whether a staircase, a suitcase or a walk are still doable.
Why it is so strongly linked to life expectancy
A study of 122,000 people from the Cleveland Clinic showed: people with very low fitness had a higher mortality risk than smokers, diabetics or people with high blood pressure. The jump from the lowest to the second-lowest fifth halved the risk, and the top fifth sat 70 percent below the bottom. There was no upper limit beyond which more fitness stopped helping. No other single value, not even a blood value, predicts life expectancy as well.
Estimating your own VO2 max
In the lab with gas analysis on a treadmill. In daily life with the Cooper test: run as far as possible in twelve minutes, the distance in meters minus 505, divided by 45, gives the VO2 max. Sports watches estimate it usably from pulse and pace. As rough orientation: 50-year-old men are good above 40, very good above 47, women good above 33, very good above 38. Every decade less of age raises the limits by about 3 to 4 points.
How to raise it
The base is zone 2, three to four sessions a week. The jump comes from intervals: once a week four times four minutes at 85 to 95 percent of maximum heart rate, with three minutes of easy rest, on a bike, treadmill, rowing machine or uphill. The Norwegian 4x4 protocol has raised VO2 max in studies by 10 to 15 percent in twelve weeks, also in the untrained and in people over 70. Whoever starts fresh begins with two times three minutes and increases slowly.
Caution and patience
Intervals are hard. Whoever is over 45, has not trained for a long time, has high blood pressure or heart problems gets an exercise test at the doctor first. Then VO2 max rises at any age: studies with 70-year-olds show gains that make up for the loss of two decades.

How often intervals?
Once a week is enough for the effect, twice is the maximum for recreational athletes. More raises injury and overtraining risk without extra benefit.
Can I still raise VO2 max in old age?
Yes. Studies with 60- to 80-year-olds show gains of 10 to 20 percent within months. The body responds to the stimulus at any age, just a little more slowly.
Is the sports watch estimate accurate?
To about 5 to 10 percent when it measures with a chest strap and over several runs. For the trend over months that is quite enough.
