Biological age: what it measures and how to lower it

Your passport says 45, your body may say 39 or 52. What biological age means, how it is measured, which markers really count and which habits measurably turn the clock back.

What biological age means

Two people, both 50, look different, move differently, have different blood values. The calendar age is the same for both, the biological one is not. It describes how far cells, vessels, muscles and organs have progressed in the aging process. A person at 50 can have a body that works like 42, or like 58. The difference decides over disease, independence and years of life.

How it is measured

The most precise method are epigenetic clocks: they read chemical marks on the DNA that change with age and estimate an age accurate to a few years. Such tests cost several hundred euros and deliver a number, but rarely an action. In everyday life simpler markers suffice, ones that correlate with mortality in studies: maximum oxygen uptake, grip strength, time standing on one leg, waist circumference, blood pressure, fasting glucose, HbA1c, LDL cholesterol, triglycerides and the inflammation marker CRP. Whoever knows these values knows more about their aging than any gene test.

The levers with the strongest evidence

Research agrees on six things. Not smoking is the biggest single factor; quitting at 40 brings back nine years on average. Cardiorespiratory fitness: people in the top fifth of VO2 max have a 70 percent lower mortality risk than the bottom fifth. Muscle strength: grip strength predicts mortality better than blood pressure. Sleep: seven to eight hours regularly, less or more raise the risk. Diet: Mediterranean, with enough protein and little sugar. And relationships: loneliness affects mortality like 15 cigarettes a day.

What you can achieve in twelve weeks

Biological age is sluggish, but the markers react fast. Twelve weeks with three endurance sessions in zone 2, two strength sessions, seven hours of sleep, 1.4 grams of protein per kilo and no alcohol on weekdays lower blood pressure by 5 to 10 mmHg, triglycerides by 20 to 30 percent, HbA1c by 0.2 to 0.4 points and raise VO2 max by 10 to 15 percent. In studies with epigenetic clocks that corresponds to one to three years of biological age.
